As part of the ongoing efforts to enhance the mental health services, Primary Health Tasmania invite you to participate in a survey for their Mental Health Continuum of Care Project. Your insights are essential in helping to develop a model of care that addresses the diverse needs of Tasmania’s communities.

This survey is a key part of an evaluation of the low intensity, short-term psychological interventions, short-term trauma counselling, and the adult complex and severe services (also known as the mental health nurse program) commissioned by Primary Health Tasmania.
Your feedback is invaluable. It will help Primary Health Tasmania to identify strengths and areas for improvement within these services and the service model they operate within. The insights you provide will directly influence the development of a new commissioning model, focused on enhanced coordination and integration to better meet community needs.
